Signs and Symptoms Themost common sign of both Hodgkin’s disease (HD) and non-Hodgkin’s lymphoma(NHL) is a painless swelling in one or more of the lymph nodes of the neck,collarbone region, armpits, or groin. But it is important to remember thatmost lymph node swelling (especially in children) is caused by an infection,not by lymphoma. Affected lymph nodes usually return to normal size within afew weeks or months after an infection has cleared. Physicians often observeswollen lymph nodes during this time to see if they change in size followingantibiotic treatment. However, if a lymph node(s) is larger than one inch insize (> 1 in), and if there are other suspicious symptoms, the physician maychoose to perform an immediate biopsy to rule out or confirm diagnosis ofcancer. If HD or NHL involves lymphatic tissue within the abdomen the belly maybecome swollen, and even resemble pregnancy in some female patients. Fluid maybuild up within the abdominal cavity, and swelling near the intestines mayblock the normal passage of stool. Such blockage may cause sensations ofabdominal pressure or pain. Otherlymphoma symptoms include the following – Chest pain Trachea General symptoms, also sometimes called B symptoms Chest Pain If lymphoma involves the lymphatic tissue of the thymus, the gland located infront of the heart, it may cause chest pain. In addition, an enlarged thymusmay press on nearby structures such as the trachea (windpipe) or superior venacava (SVC), the large vein that carries blood from the head and arms back tothe heart. Trachea Pressure on the trachea can leadto coughing, fatigue, shortness of breath, and other respiratory difficulties.Pressure on the superior vena cava may produce SVC syndrome, a swelling of thehead and arms. SVC syndrome involving the brain can be fatal and must betreated immediately. But enlarged lymphatic tissue in the chest cavitygenerally tends to displace, rather than press upon or encase, adjacentstructures. Therefore, compromised breathing and SVC syndrome are relativelyuncommon signs of lymphoma. General Symptoms As lymphoma progresses and cancerous lymphocytes spread beyond the lymphaticsystem, the body loses its ability to fight infections. The generalizedsymptoms that develop may be confused with influenza (the flu), otherinfections, such as infectious mononucleosis (“mono”) andtuberculosis (TB), or other cancers. In particular, people with HD or NHL mayexperience the following – Fever Drenching night sweats Weight loss Fatigue Loss of appetite Red patches on the skin Severely itchy skin, often affecting the legs/feet Theitching associated with Hodgkin’s disease is much more severe than that causedby dry skin. In some cases, patients may actually break the skin with theirfingernails in attempts to get some relief. The night sweats caused by HD orNHL may be so extreme that the bed sheets and blankets become drenched. Someoncologists refer to these generalized symptoms as B symptoms. Unfortunately,these symptoms are related to an increased Cancer “burden” (i.e.,larger amount of cancerous tissue) in many patients and often are associatedwith a worse outcome (prognosis). General Treatment Thetreatment of lymphoma depends on many factors, the most important of which arethe type of disease, its stage, its site (location), whether the lymphoma isslow- or fast-growing, and the age and general health of theindividual. Other factors that may enter into the treatment plan includesymptoms, whether surgery was used in staging the disease, and pregnancy.
